COMES NOW, Plaintiff, FLORIDA WELLNESS & REHABILITATION
CENTER, INC. a/a/o Jennifer Blanco (hereinafter “Plaintiff’) sues the Defendant,
STATE FARM MUTUAL AUTOMOBILE INSURANCE COMPANY
(hereinafter “Defendant”), and in support thereof alleges the following and in

1. This is an action for Breach of Contract for damages in excess of Five
Thousand Dollars ($5,000.00) but does not exceed Fifteen Thousand Dollars
($15,000.00) exclusive of interest, costs and attorney’s fees and is within the
jurisdictional limits of this Court.

2. Defendant was and remains a corporation organized and existing
under the laws of the State of Florida and is otherwise sui juris.
