Plaintiff Ms. Jennifer Lee Gilbert (hereinafter “Plaintiff’) is an individual who
resides at 7 Berkshire Circle, Ware, Hampshire County, Massachusetts.

Defendant, Allard’s Farms, Inc. (hereinafter “Defendant”) is a Massachusetts
corporation, having a usual place of business located and corporate offices at 20
Industrial Drive West, P.O. Box 259, South Deerfield, Franklin County,
Massachusetts.

FACTS

Plaintiff re-alleges and incorporates the foregoing paragraphs as if fully stated
herein.

On or about June 30, 2014, at a location on premises off of the Connecticut River
