COMES NOW, STINE LOUIS, as Plenary Guardianship of RAYMONDE LOUIS, and
petitions this Honorable Court for the entry of an Order authorizing settlement of the medical
malpractice claim entitled NORTHWEST MEDICAL CENTER, INC. d/b/a NORTHWEST
MEDICAL CENTER (the “Personal Injury Action”) and, as grounds therefore, would allege:

1. STINE LOUIS is the duly appointed guardian of his wife, RAYMONDE LOUIS.

2. The medical malpractice claim sub judice was filed against the Defendant, NORTHWEST
