PLAINTIFF’S REQUEST FOR REFERRAL TO MEDIATION
COMES NOW the Plaintiff, by and through the undersigned counsel, pursuant to the
Florida Rules of Civil Procedure, and files Plaintiffs Request for Referral to Mediation, stating as
follows:
1. Plaintiff filed the instant action alleging damages based on the negligence of Defendant.
2. Florida Rule of Civil Procedure 1.710(b) provides that a civil action may be ordered to
mediation upon motion of any party or by the court.
3. Plaintiff is requesting referral to mediation for the issues raised by Plaintiff's Complaint.
WHEREFORE the Plaintiff respectfully requests an Order Granting Plaintiff's Referral to

Mediation, along with any other relief this Court deems just and proper under the circumstances.
