Defendant, Jane Holly Butler, provides notice to the Plaintiff of a physical examination
pursuant to Fla. R. Civ. P. 1.360, and states as follows:

1. Defendant is requesting a physical examination of the Plaintiff, Mable Easterly,
pursuant to Fla. R. Civ. P. 1.360.

2. Plaintiff, Mable Easterly, has placed her physical condition at issue and in
