THIS CAUSE having come before the Court on Defendant’s Motion for a HIPAA Qualified

Protective Order to Disclose Protected Health Information, and the Court having heard argument of

counsel and being duly advised in the premises, it is hereby

ORDERED AND ADJUDGED as follows:

1. Said Motion be and the same is GRANTED.

2. In accordance with the requirements of the regulations promulgated under the Health

Insurance Portability and Accountability Act of 1996 (“HIPAA”), specifically 45 C.F.R. Sections
