MOTION TO MODIFY

COMES NOW, the Respondent, Lori J. Kastl, and for her Motion To Modify, she shows
the Court as follows:

1. This motion requests modification of certain child visitation orders contained within
the Joint Child Custody Plan filed herein on March 14, 2013, as pertains to the following minor
children: Hailey Michell Kastl born June 12, 2003, Madison Lee Kastl born March 10, 2006,
Kori Lynn Kastl born July 19, 2010, and Kodi Anne Kastl born July 19, 2010. Insofar as
pertains to this motion, said prior order has not been modified by this or any other Court.

2. Under all applicable state and federal law, including Oklahoma’s Uniform Child
