Plaintiff, Samuel Aviles filed this action against Defendants, Shadow Deliveries Corp., Howard’s Appliances, Inc., and Serta Simmons Bedding, LLC for damages arising out of a work-related incident that resulted in injuries to Plaintiff’s hand. Serta asked Plaintiff to sign an authorization to release his social security records, but Plaintiff refused.
