Come the Petitioners, Lonnie and Kari Burks, by and through their attorney, Willie E.
Perkins, Jr., and for their Petition, allege and state:
1 That a guardian of the person and estate should be appointed for the incapacitated
adult person hereunder named, whose date of birth, sex, and address are set opposite the name:

2, The above named person is incompetent by reason of mental retardation.
3 The nature, extent and value of the above named incompetent person and the
interest of the incompetent person in such property is as follows: $914.00 SSI per month
4 Lonnie and Kari Burks, whose address is 322 Broadway Street, Malvern,
Arkansas 72104, are the parents of the above named person, and are qualified persons to be
guardians of the person and estate of the named incapacitated person. That Lonnie nor Kari
Burks have ever been convicted of a felony.
5 Petitioners are at present serving as guardians of the person or estate of: no one.
6. Insofar as Petitioners are able to ascertain, they are the persons most closely
related, by blood or marriage, to the above named incapacitated person.
