                       1215 Truxtun Avenue, Bakersfield, California 93301, Petitioner will bring a Petition to approve the

                       transfer of payment rights by and between Luis Gomez-Perez, (hereinafter “Payee” or “Transferor”),
                 24
                       and Petitioner pursuant to California Insurance Code § 10134 et seq.

                                You   have   been   identified   as  an  interested   party  and   you   are  therefore   entitled  to  receive

                       notice of this action.    Pursuant to California Insurance Code § 10139.5(f)(2), copies of the                following

                       documents are enclosed for your review: (1) a copy of the transferee’s current petition and any other

                  1    prior  petition   whether    approved    or  withdrawn;     that  was   filed  with  this  court;  (2)  a  copy   of  the
                  2    proposed    transfer  agreement     and  disclosure    form;   (3) a  listing of  each   of the  payee’s   dependents,
                  3    together   with  each   dependent’s     age;  (4) a  copy   of the  disclosure   required   by  California    Insurance
                  4    Code    §10136(b);     (5)  a  copy   of  the   annuity   contract   and   a  copy    of  any   qualified   assignment
                  5    agreement, if applicable; and (6) a copy of the underlying settlement agreement, if available.
                  6             Pursuant to California Insurance Code § 10139.5(f)(2)(J)and(K), you are entitled to support,
                  7    oppose,    or otherwise    respond    to  the  transferee’s   application,    either  in  person   or  by  counsel,   by
                  8    submitting written comments to the Court or by participating in the hearing.
                  9             Pursuant    to  California   Insurance    Code    §  10139.5(f)(2)(K),     you   have   been   provided    with
                 10    notification   of the  time  and   place  of  the hearing   and   of the  manner    in which    and  time   any  written
                 11    response    to the  application   must   be  filed, which    may   not  be  less than  15   days  after  service  of  the
                 12    transferee’s notice, in order to be considered by the Court.
                 13             This   Petition  is  sought   on  the  grounds    that  the  terms   of the  subject   transfer   are  fair and
                 14    equitable   to the  real  parties  in interest.   This  Petition  is based   on  this  notice,  the attached   Petition,
                 15    upon all exhibits, records, pleadings, and papers on file herein, and upon all such other further oral
                 16    and/or documentary evidence as may be presented at the time of the hearing.
